Directions for Completing the Form
Directions for Teachers
- Where appropriate, review student learning data associated with your teaching to determine strengths and areas on which you may wish to focus this year. Be prepared to discuss this at your planning conference.
- Review the Professional Qualities and Responsibilities (PQR) completing the self- assessment and considering any indicators that may be associated with the area on which you wish to focus.
- Sign the Professional Qualities and Responsibilities (PQR) form- Give signed copy to your administrator.
- Select the specific PQR section on which you will focus throughout your observation cycle for the year.
- List the PQR section you wish to be the focus and write a short description of why you chose it and what you hope to accomplish through this focused work and feedback.
- Bring the completed Observation Cycle Planning Form to your planning conference. You should be prepared to conference with your administrator about your focus (by specific data)
Directions for Supervisors
- Verify that the PQR has been reviewed, signed and submitted to you.
- Discuss with the teacher any student learning data associated with their teaching.
- Based on this discussion, review the pre-observation conference form to determine if you agree with the PQR section of focus selected by the teacher.
- Determine if there are additional areas you would like to see as a focus. Be sure to keep this to a reasonable number.
- Add any additional areas to the principal section
- Be certain to have the teacher sign the form.
