Directions for Use:
The employee completes the first half of this report for the Implementation Conference with the supervisor (this may be held anytime between the Planning Conference and the end of the Professional Growth Series, March 1 for Probationary employees and June 30 for non-probationary employees). The employee and supervisor sign the top half of the form, after the supervisor makes comments if necessary.
The employee then completes the second half of this report no later than the end of the Professional Growth Series (March 1 for Probationary employees and June 30 for non-probationary employees) and meets with his/her supervisor to review the outcomes. Again, the supervisor makes any comments necessary, and both the employee and the supervisor sign the bottom half of the report.
